SUPRT Resources for SOR Contractors

As of October 1, 2025, the Substance Abuse and Mental Health Services Administration (SAMHSA) transitioned from the Government Performance and Results Act (GPRA) measurement tool to the SAMHSA Unified Performance Reporting Tools (SUPRT). This new reporting system streamlines client-level data collection and aligns with SAMHSA’s 2023–2026 Strategic Priorities.

The following State Opioid Response (SOR) Projects are required to complete client-level data collection using SUPRT:

  • The California Hub & Spoke System (H&SS)
  • Medications for Addiction Treatment in State Licensed Facilities (MAT in SLF)
  • Medication Unit (MU)
  • Native MAT Network (NMAT)
  • Low Barrier Opioid Treatment in Syringe Services Programs (SSP)
  • Tribal and Urban Indian Community Defined Best Practices (TUICDBP)
  • Youth Opioid Response (YOR) California

Grantees should conduct SUPRT surveys with clients receiving SOR-funded services at baseline, six-month reassessment, annual reassessment, and closeout. Surveys should be submitted through Aurrera Health Group’s SUPRT REDCap portal. There are two links to use for entering data:

  • Baseline: Use the REDCap SUPRT Baseline Link to begin a new client intake.
  • Reassessment and Closeout: Use the CA SOR SUPRT Data Dashboard Link and create login credentials using your organization email address. Once logged in, you will be able to view your site-specific dashboard.
    • The dashboard will list all clients that have received a SUPRT baseline and their corresponding REDCap Survey Queue links for reassessment and closeout data entry.

The SUPRT resources listed below are for the projects listed above and their associated sub-grantees.
